Oral Estrogen: To swallow or not to swallow.

So I've been wondering everyone's thoughts on this. I can't seem to find much regarding research about the delivery method of oral estrogen and which is better for the body and also which provides the most effective results.

I am around 10 months on HRT now and for the first 9 I have been swallowing my estradiol tablets. Recently I learned oral estrogen tablets are specially made so they can also be dissolved under the tongue quickly and with out a bad after taste (No wonder they always seemed to dissolve before I could swallow them!).

So my question here is to those who are taking Estradiol in pill form or who have taken it in the past, What method do you use and why? Obviously it's all personal preference and there is no research to support which is better or if one provides superior results to the other so I am asking you. I have not asked either of my doctors handling my HRT about this but the next time I see them I plan to.

So, from some of the research I've read here is are a couple of things to consider about the different delivery methods.

*Swallowing the pill means it is processed in the liver and may produce an estrogen byproduct which is still helpful in feminizing and transition but also that it may not be effective since it's not going directly into the blood stream.

*Dissolving the pill under the tongue means potentially exposing the estrogen in the pill to saliva which may begin to break it down before it is absorbed by the tongue membrane. This may also reduce effectiveness.

So, thoughts? Experiences?

In the last month or so since I've been dissolving the pill under my tongue it seems it has been a little easier for me to cry. Even thinking of something sad can be enough to bring tears to my eyes. I'm not sure if this means this method has been more effective for me or not and ultimately I'll need to monitor this method longer to see if I notice any other effects.

Ultimately I'm still on the fence about which method may be better for me. I've had results from simply swallowing the pills and I don't want to diminish or slow them by changing the way I take them.

I began with swallowing the E...

But after a few days, I switched to putting the tablet under my tongue..

The tablets dissolve very quickly and I can tell I took it almost immediately, not so when swallowing them...

I have had wonderful results so far, after just 5 months of starter-dose levels. I have no way to know what would have happened had I gone with swallowing the whole time, but I suspect that my results would not be as impressive...

I was at a B cup at about 3 months, if I recall correctly.

I don't know, cause YMMV, of course. But I am stickin' with sublingual, for sure! :)

I just swallow my pills, and I'm developing just fine. I'm very happy with my current rate of development and I'm filling out in all the right places. I'm inclined to believe it doesn't matter how you take them, but that's just me.

YEA! and (2) I am taking so many other pills (HRT, cholesterol inhibitor, insulin sensitizers) that maybe it helps give my liver a break by not loading it up so much.

After I had my Bilateral Orchiedectomy my Endocrinologist but me on to Estradot Patches as been the best way to deliver my HRT, and

so far they seem to work well. I have to change patches every two and half days (like Saturday AM and Tuesday PM)

So I'm thinking about trying this tommorrow when I take my Estrodiol, but it wont be less effective will it? I read somewhere in here where someone said some of it will soak up into the tongue, and it won;t be as effective. Is this true or will I be allright trying the under the tongue thing tommorrow?

sublingual every time... its safer and more effective. It goes straight into the bloodstream bypassing your digestive system and liver. Bypassing the digestive system keeps it from breaking down and bypassing the liver is just safer period.

Taking estradiol sublingually is a very similar concept to taking estradiol transdermally... with gels and patches it gets absorbed by your skin and goes directly into the bloodstream. Only difference between the two is that its the skin on the inside of your mouth that does the absorbing.

Theres a few other things... from what Ive read most doctors are starting to come to the conclusion that sublingual estradiol is the drug of choice for cross sex hormone replacement therapy. Its just as safe as transdermal but significantly cheaper. Also the pill form of estradiol (estrace, estrofem) is β-17 estradiol which is a slightly more potent form of estradiol. The only thing to be concerned with is to make sure that the particular brand that you get is not the gel coated version, that version cant be taken sublingually.... it needs to be the little chalky ones (generic basically which = cheaper anyway).

Basically its the cheapest, safest and most effective option for *most* patients. my two cents :)

I don't have the generic estrofem or estrace that your talking about, its just Estodiol in pill form; not generic. So does it still work like your talking about?

do they have a chalky consistency or do they have a shiny almost enamel like coatings... you want the chalky ones

you know its frustrating really how little research has been done into the biochemistry of cross sex HRT. The information is scattered and conflicting with some doctors saying one thing and other doctors saying the complete opposite. We need more research done into these drugs and how they really affect us. There should be less guess work in determining the best route of treatment for cross sex HRT.

taking that even further one thing Id like to see them develop, and i believe this is completely possible with current technology. I want to see someone create a dissolving 1-2 year implant that contains the right amount of each drug and layered in such a way that each drug is released to mimic a natal female's cycle. You would have different dosed versions to cover a range of people; you wouldn't be able to fit 100% of patients with it but you could make enough versions of the implant to cover at least 80-90% of patients.

I think it would be awesome but a project like that is unlikely to get funding I'm afraid...sigh

Best chance is that the body knows a bit, and considers the E itself worth processing directly, takes it, and lets you swallow the sugarlike non medicine after the fact.

WPATH did acknowledge the lack of cross gender hormone research last fall, and there are groups within it, that will do the real research themselves, or find others who have. It is an area of high concern with them. The research element is made all the more necessary by the DYI crowd who post dangerous information on the Web, and cause problems for the care givers when they prescribe one dose to a patient who thinks they are being ineffectively treated, and then the caregiver is reviled for incompetence for low doseage stuff.

That helps make things a little easier as far is my confidence with taking the pills sublingually.

The problem with medical research is that it needs funding and since most trans people are struggling to survive and pay for surgery I doubt there are many large organizations or significantly wealther individuals out there waiting to throw money at research involving our community's health interests.

I can say that in what little amount of time I've been taking my Estradiol sublingually I've finally developed tenderness in my breasts. I hope this means they're going to start growing again soon! Especially since the left one is significantly smaller than the right... Keeping my fingers crossed for at least a C to help offset the broader shoulders.

And yeah... the sugary bit doesn't always dissolve completely especially if I take the prescribed number of pills and try to dissolve them all at once which I normally don't do unless I'm pressed for time.

Well, I didn't mention that I started putting the Estrodiol under my tongue a few days; I really know that I'm taking it,well maybe it's all in my head, but I was wondering if I should do the same with the Spirolatone?

I don't know about you, but my spiro tablets are pretty big. It would take half the day for them to dissolve under my tongue! ^_^

I admit I've decided to try it with the estrace, though, just to see if I notice a difference.

I read somewhere that spiro under the tongue can cause a sore or something, but I don't recall the details, sorry!

I put E and Progesterone under my tongue and swallow my spiro...

Oral Estrogen is made to be dissolved quickly and without a bad taste under the tongue. Well, certain types of oral estrogens. If it's gel coated or something then it's meant to be swallowed but as I recall all generic forms of oral estrogens, in the US, are designed to be taken sublingually.

Spiro probably wouldn't taste good taking it this way and as Shilo said it would probably take forever to dissolve. I would stick with swallowing the Spiro until you can consult your prescribing physician if nothing else.

btw you should not take spironolactone sublingually its gross for one thing and it could lead to an overdose. Estradiol sublingual is ok though but you should still always consult your doctor before changing anything.

      Good morning,   I over slept yesterday was a couple minutes late clocking in.  But no breakfast or coffee.  Got caught up but it was go go go all morning.  I had to ask for a refresher on how I was to enter something but once I got a quick answer it came back to me.    @KymmieL sorry Ford didn’t work out.  We are always looking for good reliable people, I could get you a job here but the commute would be rough.  Today I have three audits to get done, plus other things during my shift on top of the regular things.  Since I am opening that puts me in the drivers seat.  The Asst Mgr comes in part way throuh my shift but she will have to handle customers while I do the work she would be doing if she opened. Tail wagging the dog.  Guess she’s getting punished for not following the Mgr’s requests.  They do tend to butt heads a lot.     Butting heads with people is a thing the Asst is known for.   @awkward-yet-sweet do you think just maybe this new graphics request was in the offing?  And why you were asked to go to work with your husband?  Obviously, he cares about you a lot and is trying to do things to help you.   speaking of meeting people @Adrianna Danielle we have a youngish customer who comes in frequently, I’d like to approach her but I’m just not certain yet.  She still dresses male but has long hair and early chest development.  My approach, if I ever decided I should would just be supportive but I really can’t be sure that is what is going on here or what and I would not want to make a big blunder if that’s not what he is doing.  A male with early teen boobs doesn’t want to be noticed.   well, I can’t be late again, I’ve got to leave now.  See you again later for afternoon tea and crumpets or scones.       Well, HIPAA is basically useless for keeping government out of your medical stuff.  It doesn't seem to prevent employers from making workplace medical demands either.  About the only thing it seems to do is keep somebody's sister or spouse from having the tools necessary to help you when you're in trouble.  As usual, government made things worse and added unhelpful red tape.  I really doubt HIPAA will be any use in the area of trans rights either.    Honestly, I don't see anything good will come of this no matter how it goes.  If some state AG's win on this, it will cause issues for trans folks.  If the Feds win on this, it'll be a precedent to stomp on states' rights even more than has already been done.  And I'm not sure which way things go will make a difference when it comes to officials from one state trying to do nasty things to people who have left that state and gone elsewhere.    What a crap sandwich... and no matter which plate it gets served on, "We The People" get to eat it.

      At least you tried!  Something equivalent or better may come up, and the waiting could be worth it.  Just keep trying and you'll eventually get what you need and want.      For me, having somebody to love was the most important.  Everything else follows after that.  I waited a long time to find somebody...and she ended up leading me to more than I ever thought possible.    Actually, I'm feeling pretty good right now.  I have something work-like outside of my home responsibilities to do for the first time in about 18 months.  Nobody seems to mind the real me.  And this evening, my husband said something that just really made me feel special.  He was rubbing my back, shoulders, and chest while we talked, helping me relax.  He told me that he thought I was really cute in my girl form, but that he thinks my boy form might even be cuter.  And that he's proud of his "smart little Pocket Fox."    For me, the combination of those sweet words and the physical affection was exactly what I needed.

      Tuesday night.  They had a quick supper together at a fast food place.  Bob went off to teach karate and Taylor locked herself inside her apartment and worked on her hiring plan.   First the web site problem.  The two guys who ran it were self-taught and knew little.  It currently had three pages, the Home page, the About page and the Contact page,  She asked them to work with Karen in terms of redesigning it and she needed three designs to show Gibbs tomorrow.  The problem was three fold: the two guys and Karen.  Millville was a small town and all three were relatives of members of the Board.  Millville, Millvale. She was doing it.  People here called it either way, sometimes in the space of a few seconds.  She thought it was Millville.  All three had complained about the work, because the two boys regarded it as done and untouchable, even though they actually had not worked on it at all for months.  Like a number of people, they showed up and collected generous pay checks and did nothing.  She had looked at a number of websites and she had been told the company wanted one both internal and external customers could log into.  Her chief difficulty at the moment there was that there was very little content.  She decided to send the three complainers out tomorrow to take numerous pictures of the thirty acres  Or was it forty?  No one seemed to care. She cared, because she needed to get it right.  She debated outsourcing the website to a company, but first she needed something to outsource, and before then she needed to decide whether to keep these people.  She didn't need to mess with them.  So she decided to recommend they hire an experienced website developer with management skills. Would such a person come to Millville?  The schools were good, because the company had poured money into them, and the streets were well paved.  The company had bought all the abandoned houses and maintained them, hoping someday they would be filled again. Millville was crime-free.  People did not lock their doors. Neighborly. Very conservative, but in a good way.  Hard working, ethical, honest. Maybe the Chinese money was corrupting the town?  Not sure.  So she thought they would hire someone, even if it were a remote position.  She would rather have them here, but she would take what she would get.  That would move the website out of her hair. Secondly, she needed an effective presenter.  She could not do all these presentations herself.  She had natural talent but a lot could be passed on. She needed another Mary and another Brenda, or their understudies, effective hardworking people.   Bob. Was he okay with this?  He said she was Management.  Was that a problem?  And she was now earning a ridiculous salary, which she put down to company dysfunction more than anything she had done.  Was that a problem? She was not sure.  He was highly competitive and he had that male ego.  She did not.  A feeling of guilt rose.   Her therapist had brought up her feelings of guilt about not making Dad's expectations, never being the man Dad wanted her to be.  She never could, and this physical evidence backed that up.  What would the doctor say?  She thought about it, and that her therapist said she needed to find a sexual assault survivor's group more than a transgender group right now. Was there one here?  She thought about serving in a women's shelter.  There was one here, oddly enough connected to the church they had visited.  That F on her drivers' license would help.  She was waiting until after she talked to the doctor again to move on that stuff.   Was Bob really buying 160 acres near the old air strip on speculation?  Much of the land around Millville had been for sale for a long time.  That land was being offered at a dollar an acre, the owners having inherited it and now living out of state. Common knowledge.  They would take the first offer, and it had been for sale since the airstrip closed twenty years ago. Airstrip.  That would help.  Not tonight. Focus, girl, she told herself, and read over her notes to do so, which were making less sense the further down she went. It was eleven, and she gave up and went to bed.

      .my name is violet. I'm new here and thus is my first try at forums. I'm 45 and just recently having came to terms of who I really am. Thought a lot of self discovery since I stopped drinking. Drinking was my coping mechanism to hide a lot of thing. There were plenty of signs though the years. As I look back. That i hid inside. Now really sure what made all of this bubble to the surface at this time in my life.  Mabye it was waiting for me to be open minded and ready to accept that I am trans. I have a very unhealthy environment at home that is anti trans. I really don't know what else to say but hi. I hope everyone here will be accepting of me and me work through my journey of finding the real me. I know that since I accepted it I have been much happier than I can remember. Being to real me makes me happy. I hate having to hide this all the the time at home. I work retail management and have no idea if I could even stay in this business if I am to fully come out. Wow that was scary saying all that. It's a first for me

      Thanks.  What I do as a man is what a woman would do if she were a man.  There is just something feminine about the way I act as a man.  It's not that being a woman is actually better, or something to aspire to, but it is just that I am one, while not being one.   If beating my head bloody to get rid off this stupid dysphoria would fix it I would find the nearest wall, but I know that if I did that, when I woke up, it would still be there.   If I did not have this struggle I would be someone else and I would be less of a person than I am.  They say an oak tree growing in an open field is far stronger than one in a forest.  The storms come and go and I stand.   This forum is the first time I have interacted with other people struggling with the same struggle and parallel struggles. It helps.
