There's a cross-dressing/transsexual anime out called "Natsu no Arashi!", where ghosts exist and work in a cafe. Jun-kun is the cross-dresser because he is really a female model, who wore the male outfit that she was modeling to work on her first day (at the cafe), and giving the owner the impression that she was a guy! Later on, she falls down the stairs to a temple with another employee (a boy named Hajime-chan), and they end up switching bodies!

No Bra (のーぶら, Nobura) is an interesting manga. It didn't state in wikipedia about whether the main character is a crossdresser or a transsexual, but read the manga and it will be obvious.

I am hooked on it. The main character is soooooo pretty!

OMG!!! I just finished all of them and was kinda happy and upset about the Ending Maa-kun should have ended up with Yuki.

One of my favorite anime series is Steins;Gate. It's mostly about time travel and how the decisions we make impact the lives of others, but one of the supporting characters is transgender. She's MtF. There's actually an episode where the main character does something to alter the timeline in such a way that, when he returns to the present, she's biologically female. I'm not going to spoil it, in case anyone wants to see it, but it actually ends up being quite a tragic episode...

I don't have much to add to all the anime listed here. But this topic just reminded me of how Ranma 1/2 has always been one of my favorite anime, and my room mates and most of my friends were always like "But... Ranma is terrible >.<"

And then a couple days after I came out to my room mates I was like "So, my reasoning my liking Ranma makes a little more sense now doesn't it?" And they were all like "I get it now!". It was kinda funny. We all have similar tastes usually, so they were always confused by my love for Ranma.

Ya... Hourou Musuko/Wandering Son is a great anime/manga. The drawing for the anime is very beautiful. Deals with a boy who wants to be a girl and a girl who wants to be a boy. Very touching story with a few hardships along the way. I know I can relate to the anime and the emotional aspects that it hits. No fanservice either which I find to be enjoyable. It's just a really good coming-of-age story.

Well there's also Queen Ivan Kov and dear Bon Clay from One piece. Dam I wish that hormone devil fruit were real, would be soo amazing.
