My first Trans MD appointment

Background:  I found a Trans MD in Central NY about 90 minutes from me and called to make my first appointment.  She called me back and asked a lot of basic questions - things like do you smoke, who is your internal medicine MD presently,  when did you realize you were a woman,  what kind of insurance do you have,  who does your bloodwork,  who is your pharmacy, etc.

Question 1:  What should I expect from this first meeting?  I am sure she will go over things with me some more;  but can I ask her about when I can start HRT or will she lead that direction?  I really want to start HRT as soon as possible and I also want to talk to her about an orchiectomy and therefore not needing Spiro.  I heard Spiro has some potentially deadly side effects like DVT and heart disease. 

Question 2:  Should I be scared of an orchiectomy?  I am a big baby and afraid of surgery.  I am actually afraid of my own shadow - I am kind of a scaredycat in general. 

Anyhow - any advice would be greatly appreciated!

Question 1: I'd ask for HRT. I'd also mention if you have been in therapy or not. They may do informed consent, they may want a letter from the therapist.

I told her about my therapists from elementary school up through present.   I took some notes of things I wanted to talk about.  I am of course very excited and counting the days.  The two things that are very important to my emotional well being are HRT and an orchiectomy;  and my wife supports me on both.

I can't comment on what the Orchi is like, but I have decided to not have one at the moment. I've read that if you want bottom surgery you need as much material down there as possible, after an Orchi things shrink even more and they end up needing to do a skin graft. I'm in my early forties and in good health, so I have a little time before Spiro becomes a problem. Hopefully by then I'll have enough money for the bottom surgery outright.

Just thought I'd mention this so that you know all your options and you can make an informed decision going forward.
