Are transgender surgeries a tax write off

Gender confirmation surgery (GCS) and HRT definitely yes, as well as bottom surgery for FtM's.  Breast enhancement for trans women, and mastectomies for trans men may be deductible, but you'd have to show documentation from a doctor that the procedure was medically necessary.  There are caveats to this, as with any medical deduction; the cost of the surgery or treatment must exceed a $$ threshold based on your taxable income, and you would need to qualify to itemize your deductions.

As far as I know, FFS has not been ruled by the IRS or the U.S. Tax Court to be an allowable deduction, but there are always exceptions.  I do believe some folks have been successful with that.

It is a Schedule A item for medical expenses.  It must be over 6% of your gross income to be used, and you deduct it from your gross income which determines your tax rate.  Go to a licensed Tax Preparer and let them handle it for you.  Same is true for most state income taxes as well. Your local LGBTQ center will also usually have tax volunteers trained by state and Federal Tax agents.  If you do not know how taxes work, go to help center or go online for instructions.  I had a 33 jyear career as a tax agent and we do not mess around.

 

Quick example for you:

$68, 725.00 income = $11,059 tax.  ==   , Surgical Expense =  $15,500  (22% of gross income)  reduced income to  $53,255.00  = Tax of $8,375 or saving of $2,375 in taxes. (Done as an example from the 2019 IRS tax tables.)
