Seeking VA & Wpath process guidance

Hi, I'm new to this site and have a question about the VA process and how it relates to Wpath.

First off, I have been in transition for way too long. I began seeing a less than qualified GT in 2004 but my civilian insurance would only cover two visits and only if classified as depression. Again, in 2011, having a good job, I saw a caring and experienced GT for nearly 3 months until my job ended and so did my insurance. Now, about a year into the VA health care system, which I think is a superior system, I want to continue and, hopefully, fully achieve my transition. My question is this: Should I start by requesting an appointment with VA Mental Health and get back on the GT track? Or should I just request an appointment with my PC for hormones? Although I had not received a letter from my civilian GT in 2011, she did support my discussions with my then GP and my search for an endocrinologist.

I really need to get going on this as I don't want to face the depression that is brought on by discouragement. I would surely appreciate you help and advice. BTW, I am served by the Palo Alto VA Med. Center.

One added comment. I am very proud of my military service both active and reserve. I have also, as a college professor, over the years sought to help veterans returning to the classroom and active duty students. I love the military family and am so pleased to have found this site.

Hello AlanaMarie,

I am also a veteran and beginning the transitioning process. Last Friday I had an appointment with my primary care doc (Standard Diabetic Appt), while there I inquired about seeking a gender therapist. While she said that she wasnt aware of any local VA specialist in my area, she was willing and did put in for a mental health consult to a phycatrist (SP). This can lead to a consult and maybe a fee based arrangment to a gender therapist.

Now im a 60% disabled vet already, all my medical care is pretty much covered by the VA. I found that when I brought the subject up, everyone (IE nurse, intern, and Primary care doctor) was highly professional and courteous. I havent recieved my appt yet, but I am looking forward to it. The fact that they were already routing me into a chain that could hopefully take care of me better right off the bat is a good sign. I hope.

I actually had a very big fear of them pointing at me saying you want to be a woman. After 15 years in Submarines, I have more than a little bit of social anxiety. (My mind happens to be very mean to me)

Anyways, my suggestion is just to ask your primary care doctor for a referral or consult for a gender therapist. IT went a ton smoother than I expected.

Thanks for your guidance Heather and Mackenzie. I will be calling my PC on Monday and requesting an appointment to discuss HRT. I'll let you all know how it goes.

I made the call this morning to schedule an appointment with my PC to discuss HRT. I'm excited and can't wait to meet with the doctor.

So to keep everyone updated, my PC called back right away. She planned to schedule me with the endocrinology department. By mid afternoon, I'd received a call detailiing the blood tests and bone density scans I would need and other elements of the process like the concent agreement and visit with the psych before hormones will be prescribed. This will be a weekend of excited anticipation for next week's progress and my report back to you. Have a wonderful weekend.

I am in the VA system, and have been taking Estrogen since 1971----that was with a civilian doctor until VA Picked up my care. I meet with head of the Endo department in Denver, and she refuses to authorize my estrogen----she says she is only going to do that if I am in the sex change progem, have worked with a therapist, and have lived as a woman for one year-----

I have spent years with a qualified therapist, and do not plan to have surgery, I just need a script for the estrogen. I even told her I can get the Estrogen filled in Canada much cheaper, (same exact drug), and so I really just need a written script----so no cost to VA.

She flatly refused, and storned out of the appointment----

My questions is, where do I go with the system from her to just get a script written?

Hi Julie,

Ummmm, I'll take a shot at answering your question since I have spent about a dozen hours reading the threads in here. You should go see your Patient Advocate and also take the Directive http://www.va.gov/vh...asp?pub_ID=2416 or VA Directive 2011-024. . I talked with a person personally in the 202 area code who stated that the directive dealing with tg hrt will be renewed right about the time of expiration. If that does not work, then you may have to contact lambda legal or your nearest LBGT center which has legal services, since they are not following a VA Directive. That should put fire their behinds to an extreme.

Wow, I feel wonderful! Had another good experience at VAH today. Found out my medical records indicate that I am TG. That is a wonderful feeling in that the providers are full aware of who I am and where I am in transition. That makes me feel a whole lot more confident. I no longer have to explain myself to each provider as I did with previous company provided healthcare. And it's nice to be able to be upfront and learn how providers care for those of us who are TG. All the people I've seen this week have been very polite and respectful. I truly appreciate the VAH professionalism and respect for my dignity as a transwoman.

I just wanted to share that with you and I try to keep you posted on my experiences.

Another update: I've met with endocrinology and discussed my plans and goals. They took their time with me as we discussed risks and benefits of HRT. I also signed the consent document and will meet with the psychologist for a psychological assessment very soon. After that, I should receive my first doses of hormones in the mail.

I am excited for the first dose of estrogen and am really looking forward to those physical changes that will prompt me to live full time in the near future.

Wooooo Whooooooo!!!! I am so excited and very, very happy. Had my psych assessment today, the last stop before the HRT meds arrive. I passed with flying colors. I feel so good right now I could dance, no, shop, no, dance. Yes, I think I'll dance a very happy dance. We had a wonderful discussion that focused on a positive outlook for my future. "I am woman!"

So I share my excitement and wish you all a very happy day and a Merry Christmas filled with love and joy.

      My condolences to the people of WA.  A huge warning about that type of politician though, those who broadcast the Anti-Trans message rarely if ever attend to any important legislative issues to benefit their constituency.  They are mostly single trick dogs, whose trick is to empty their food bowl and whine for more food.

      Exactly.   A plain old "Good morning" should always suffice, for example, whether the recipient expects a gender specific greeting or not.

      This sort of stuff makes my blood boil. A prospective state politician has gained approval from his party's state council to run in their state election next year.   He once stated that the + part of LGBTQIA+ is where pedophiles are categorized.   For clarity, in Australia the Liberal party is our main conservative party. They have lurched further to the right over the past 10 years or so. Our main progressive, union backed party is Labor.   WA Liberal party approves Dr Thomas Brough, candidate who linked LGBTQIA+ community with paedophiles - ABC News

      Happy Mother’s Day    I have to be at church earlier than normal for a mic check.  I have been going over what I am to say, both silently reading and reading it aloud.  My concern is that my voice will freeze as it sometimes does.     I felt I was honored very highly yesterday by my minister and supporter on my journey.  He and another minister are Standing together to become the next co-moderators of the Presbyterian Church (USA).  They included me to receive an advance copy of their initial submission.  I was one of only 6 people listed on the email to receive this.  Later this week it will go out to all the voting commissioners that will be in Salt Lake City for the meeting.  I have asked to be allowed to attend the meeting to support him and Rev CeeCee Armstrong, whom he is standing with.    We’ll time to get ready .   Willow

I'm 61 and haven't had a birthday party since I was 30. Even that was just a crummy meal at the pub with a few friends who we have since lost contact with. Don't sweat it.

But there is one thing you might have neglected to say at the end, and that is, "And they all lived happily ever after!" ❤️   Reno sounds like a pretty good place to call home. I understand the attraction of living in a place that is not too big but not too small.   I look forward to reading your subsequent posts about specific aspects of your journey.

      Hi and welcome. A few things...   1. You're right, you can't trust your friends w/r/t the question of passing. How would they even know if you pass or not? My sister used to do the same thing, telling me that no-one knew I was trans and that to her I just looked like a cis woman. I still don't know if she was serious or just trying to make me feel good, but I learned to not listen to her on that topic.   2. Buuuut, having said that, I thought I looked good, and I think that is all she was really trying to tell me, that she liked how I looked as a woman. Maybe this is obvious, but I feel it gets overlooked: passing and looking good are not the same thing, not even remotely. Your friends (I presume) are trying to tell you that you look good as you are.   3. Having said all that, I think non-passing transmascs have a harder time than us non-passing transfemmes. All I have to do to get gendered correctly 95% of the time is wear a dress and makeup and only a few seriously ignorant folks and diehard transphobes misgender me. For you, it's much harder to find signifiers powerful enough to send the same message.   So in short, I don't have any answers, except that you're right, it sucks, and I hope the situation improves over time. But passing isn't everything. Maybe you're right, and in a less conservative state it wouldn't happen so often, because you'd have more people like you surrounding you. I know that where I chose to live while transitioning has been a big part of making the process a positive experience. I'd hate to be in Texas atm.
